NOTE: Your cancellation request will be processed on the day it was received. You may not request a cancellation date prior to the date of the submission of this form. Radio equipment, POE, and/or Modem is the sole property of NWIC and you the subscriber are fully liable for equipment until picked up; therefore, the radio will be picked shortly after the last day of required service. NWIC will refund any pro-rated charges upon successful pickup of indoor and outdoor equipment. If no-one is home at the time of pickup it will be the responsibility of the customer to return the indoor equipment.
